1. Closing date is 10th May 2023. Entries made after this date will not be considered.
2. Successful applicants will be chosen at random and notified by email.
3. Following the confirmation email, successful applicants will be contacted via the telephone number provided on the application form. Three attempts will be made to contact successful applicants by phone.
4. Following three unsuccessful call attempts an email will be sent to the email address provided on the applicants application form.
5. Within 14 days of the above email being sent, if a successful applicant is uncontactable on the contact information provided by them on their application form, they will forfeit their opportunity to receive free treatment. In this instance, another applicant will be randomly selected.
6. All successful applicants must complete the Informed Consent process as per normal protocol and are subject to our standard laser eye surgery terms and conditions. This includes attending a free consultation at an Optical Express clinic to determine clinical suitability for laser eye surgery, as determined in consultation by an Optical Express Optometrist and Ophthalmic surgeon. Consultation appointments will be booked between Monday–Friday.
7. All successful applicants must bring and show valid ID for their profession in either NHS, Fire or Police emergency services. Optical Express reserves the right to withdraw offer of free treatment to any applicant who does not provide satisfactory ID to the company.
8. The final decision for treatment rests with the Ophthalmic surgeon and non-candidates will not be treated. Optical Express will not enter into correspondence for applicants who are not deemed clinically suitable for treatment.
9. The treatment offered to applicants deemed clinically suitable is free and no purchase is necessary.
10. The treatment offered will be our most advanced laser eye surgery. The total value of all donated laser eye treatment is £1 million. Individual treatment value is dependent on clinically suitable candidates’ prescriptions. No cash or other alternatives are available.
11. Treatment must be carried out by 31st December 2023. Successful candidates will be offered a selection of dates for surgery before 31st December 2023. Treatment dates will be offered between Monday-Friday. Any treatments not booked and carried out before this date will be deemed forfeited.
12. If treatment is cancelled by the successful candidate they will forfeit their opportunity to receive free treatment.
13. Only the named applicant who entered the Thanks A Million initiative can be treated and treatment is non-transferable.
14. If an applicant fails to attend their initial consultation without informing Optical Express and rescheduling their appointment they will forfeit their opportunity of free treatment.
15. Optical Express will not pay for or reimburse any costs or charges associated with clinic appointments before and after surgery and on treatment day itself. This includes but is not limited to; travel, parking and hotel stays.
16. Post-surgery you will be required to provide a video testimonial which will be used alongside your photograph on our website and in other PR activity. Please email video testimonial to marketingsupport@opticalexpress.com no later than 3 months from surgery date. Optical Express reserves the right to invoice the total cost of treatment to any applicant that does not provide a video testimonial.
17. Successful applicants consent to their stories, likeness and imagery featuring on our site and in PR activity. All successful applicants will be featured on the campaign website, including their name and photograph.
18. Optical Express Ltd reserves the right to review and revise these rules without giving you prior notice. By booking a consultation as part of the next steps of our Thanks a million initiative, you agree to be bound by these terms and conditions and any changes as and when they occur.
